Update `FormFileModelBinder` to re-add prefix `ParameterBinder` removed incorrectly
- #7562 part 2
- add `OriginalModelName` to `ModelBindingContext`

nit: take VS suggestions, mostly to inline collection initialization in `FormFileModelBinderTest`
