we will look for a developer certificate on the file system if a
password has been specified for the Development certificate.
* We will look at ${APPDATA}/ASP.NET/https/<<AppName>>.pfx for windows
and fallback to ${HOME}/.aspnet/https/<<AppName>>.pfx
* In case the password wasn't specified through configuration, the file
is not found on the file system or can't be loaded, we won't
do anything.

* Adds launchSettings.json to all of our templates.
* Provides an alternative flag to exclude it.
* Adds HTTPS support in launchSettings.json.
* Re-adds HTTPS redirect and HSTS middleware for C# templates.
* Improves some infrastructure aspects:
* Prefix all our test projects with AspNet.Template to allow easy
killing of orphaned processes.
* Setup an HTTPS certificate automatically for use with tests in the
same way our tooling does it on customers machines.
* Switch from OS selected ports to randomly selected ports. This is
required as we need to know the port to use for HTTPS.
* Allow disabling browser automation through an environment variable.
* Setup Firefox to allow untrusted certificates when using Selenium.
* Allow running Selenium tests on firefox by setting an environment
variable.

Support the Extensions.Abstractions from Hosting.Abstractions ( #1259 )
...
- The goal here is to enable components that use hosting abstractions to use
the web host. It lets us start to decouple components from the web host abstractions
where possible while not breaking any existing components. This will allow things
to work in both the generic host and the web host. The one snafu is the WebHostBuilderContext
which has an IHostingEnvironment typed as the AspNetCore.Abstractions type.

Use ipv4 when binding to ANCM port ( #470 )

- Today we use localhost which binds to both the ipv4 and ipv6 addresses.
ANCM never communicates over ipv6 so it's pointless. This should also properly crash
the process if the ipv4 address fails to bind (which we've seen reports in real applications for).

Add Template abstraction
...
This change adds the Template as a top level abstraction. URL templating
is now a two-stage process.
First you use a 'key' to look up a Template, then you use the Template
to create the URL.
This change also has some cleanup of the way RoutePatternBinder gets
instantiated. I added a factory service so that most of the complex
things can be made internal to Dispatcher. Now it's much easier to
constuct and use. These impacts some pubternal APIs that we already
broke, but makes them actually nice :)
Also cleaned up some tests and fixed one that was broken and not
running.

* Add BackgroundService, a base class for long running HostedServices
- Today the IHostedService pattern is a StartAsync/StopAsync pattern. Neither of these
methods are supposed to return a long running task that represents an execution. If
you wanted to have some logic run on a timer every 5 minutes, it's unnatural to do so
with simple async idioms. This base class implements IHostedService and exposes
a pattern where a long running async Task can be returned.
- The token passed into ExecuteAsync represents the lifetime of the execution.
- StartAsync and StopAsync were made virtual to allow the derived type to
indicate Start failures.
- Added tests

This refactor introduces two major changes
1. Now creating the 'handler' delegate happens inside the endpoint
middleware. This allows you to short circuit even harder, AND to create
endpoint funcs that capture and use 'next' to rejoin the middleware
pipeline.
2. Relayered the implementation to have routing plug into the dispatcher.
It wasn't immediately apparent to me that this was the right thing to do,
but I think we will need to do things this way to deliver the kind of
back-compat experience we need to do.
The idea that I have is that 'attribute routing' will be the 'default'
entry in the dispatcher. Adding additional conventional routes or other
IRouter-based extensibility will be possible through adapters - but the
default experience will be to add items to the 'attribute route'.
So. We will need to port the attribute routing infrastructure to the
dispatcher library.
We may also need to make RVD into a subclass of something in the
dispatcher assembly.

- Use the pipe events and removed the Tasks from ConnectionContext
- Remove OnConnectionClosed from FrameConnection. Since the `FrameConnetion` is a single middleware, not the entire pipeline, we shouldn't need to wait on the connection close there.
- It seems like the callbacks are rooted on the pipe even after they fire. This needs to be investigated in pipelines.

- WINDOW_UPDATE frames are validated
- CONTINUATION frames are sent for large header blocks
What doesn't work yet:
- Flow control in either direction
- It's not possible to encode a single header across more than one frame
- Affects only a very large header (name and value combined ~16KB long)
- Request trailers
- Response trailers
- Limits and timeouts in `KestrelServerLimits` are not enforced on HTTP/2
- HPACK use is very limited on the send side
- Literals are not Huffman-encoded
- Common headers (e.g. "server: Kestrel") are never indexed
- Honoring client settings
- Some error checking is still missing (e.g. validating incoming frame size)

Added Timeout to HttpClient to always be longer than the InvocationTimeoutMilliseconds.
...
HttpClient defaults to 100 seconds. If InvocationTimeoutMilliseconds is greater than the default of the HttpClient, the task will fail with a Task was canceled exception.
