This change adds support for accepting a namespace name in extensible
directives. This will be needed for the @namespace directive.
Implemented new parsing and codegen for namespaces using nameof().
Also fixed any issue where we were not global::-qualifying object where it
was used in the design time code for tokens.
- Keep all the code in the test itself, instead of in an
extension method. Makes it easier to understand what's
happening without having to look up the extension
method's definition.
- Prior to this change default imports would get line pragmas generated for them because thye'd have a source location but no file path (they were a dynamic document).
- Re-generated C# files to reflect new line pragma changes.

- Put everything in the libuv transport package under `Microsoft.AspNetCore.Server.Kestrel.Transport.Libuv.*` namespaces.
- Move stuff in Transport.Libuv/Internal/Http and Transport.Libuv/Internal/Infrastructure to Transport.Libuv/Internal (keep the Networking directory for the libuv wrappers).
- Add `Libuv` prefix to most libuv internal classes.
- Rename `KestrelEngine` to `LibuvTransport`.

- The lazy addition of namespaces gives the main document lowering phase an opportunity to add source location information which we then add after the main lowering.
- Re-generated csharp to capture addition of using statements that were previously overridden by defaults/imports.

- Today we don't end the request if the application completes
but the websocket transport hasn't gotten the receive frame as yet.
This changes adds a WebSocketOptions.CloseTimeout to EndPointOptions
that allows configuring this timeout. When the timeout is reached, we abort
the connection and end the transport task so that the request can end.
- Added tests for websocket timeout and skipped tests for application timeouts
