Use pipelines for SocketOutput
...
- Changed socket output to be based on pipelines
- Changed connection filter glue to be based on pipelines
- Codegen that used `MemoryPoolIterator` for output now uses `WritableBuffer`
- Made `UvWriteReq` async/await friendly with `LibuvAwaitable<T>`
- Deleted MemoryPool and friends
